Procedure to request an air ambulance evacuation

One call will give you quick access to our Flight and Medical Departments.
Please be ready with the following information:
 
  • Caller’s name and call back phone number.
  • Patient’s name, date of birth and diagnosis.
  • Name and phone number of the hospital and the location of the patient.
  • Referring physician’s name and phone number.
  • Requested time and date of evacuation.
  • Receiving physician and hospital names and phone number.
  • Information of family members attending the flight.
  • Social Security number and Health Insurance information (if applicable).
  • Major credit card if Health Insurance is not available.
